Seizure Action Plan and Medical Alert Form (Form)
Parents/guardians and the student’s primary practitioner should use this form to inform the school about seizure care. Update it yearly or after any seizure at school, or changes in the student’s condition or treatment.
Request for Seizure Intervention Training Form (Fillable Form)
NSS trains non-medical school staff to perform physician-ordered seizure rescue interventions for students from kindergarten to grade 12, following the student’s Seizure Action Plan (SAP). The rescue order must be dated within the past year. Parents/guardians must review the SAP with the school during a Parent Information Sharing Session.
School Checklist (Fillable)
School staff must complete each checklist item before providing seizure rescue intervention to a student. The designated school official will verify completion and readiness to administer student-specific intervention.
